Error on "Accept terms" form submission

I am getting an error on the "Accept terms" form submission code located that I downloaded here:

The errors is

document.forms.agreeform.agreecheck has no properties

It specifies line 497 however there is no line 497 on my page. I copied the code from the site but if you want to check my page you can do so at

The link doesn't seem to be working. I would suggest debugging issues like this with Firefox's Web Developer tools-- it comes in really handy.

As far as the line number goes, it is probably including an embedded file in the line count. For example, <script src="file.js"> would increase the number of "lines," as though the external file was actually part of the webpage (I could be wrong on that, though). The previously mentioned tool will show you the exact line, if you can't find it yourself.
