Looking for php form input and display script

I am looking for a simple php script, where I can fill up the form in one page and display it on the another page from database. Is there any reaymade script like that or any software I can buy ?

There won't be a "ready-made" script for your needs as it all depends on your database setup and the format of the output that you want.

However, this is a basic part of PHP-database interaction and you should be able to find tons of tutorials on how to write the script yourself.
