Autofilling textfields in form when selecting an option from drop down

Hi,

i would like to have a drop down with options: Steven, Juan, Carlos.

when Steven is selected the textfields below will autofill with his info
the text fields are:

name:
adresss:
phone:

it will do the same for juan or carlos

can you guys help to figure this out?:confused:
