Recall Form Values script 1 - problem after submitting form

3) Describe problem:

I am having trouble with this script, I have placed it into my form following the instructions but everytime I submit the form then go back to the form I get the word "Undefined" in the text boxes.

After the form is submitted, users get taken to a Thankyou page which has a button to go back to the form to submit details again.

I am assuming that the problem it is somthing to do with users being sent to a thankyou page before going back to the form.

Any ideas???
