Error when opening this page with IE-What's wrong?

Hi,
Could anybody tell me what may go wrong with my page when opening in IE6.
http://www.mindcatchingdesign.com/radford/survey_bcr.php

The page still displays, but with an error popup, and an error message on the status bar of IE saying:
"Opening page res://C:\WINDOWS\system32\shdoclc.dll/dnserror.htm..."

I used php just because want to reuse nav, footer, header. No database involved yet.

I get a different error:
Internet Explorer cannot open the Internet site http://www.mindcatchingdesign.com/radford/surver_bcr.php.
Operation aborted

So.... not really sure what's up with that.

It may have something to do with a javascript on the page. This isn't about PHP.

The page loads fine then tries to, I guess, forward to the next page, but IE aborts it. Not sure what's going on.
