Form Processor

Does anyone know of a free php form processing script that would allow me to set

some fields as required, can both send to an email address and populate a flat

file (something I could export to Excel) AND is easy to set up?

i am not sure about setting up to send to an excel spreadsheet but just follow the top quick installation and you should be able to specify all the fields that are required and setup sending yourself an e-mail in no time
