Polling Script

Does anyone know of a script that will allow me to put in a simple poll and have the results display below.

Basically I want something like: Do you want choice 1, choice 2 or choice 3

After you hit submit it would put the results so far and would include some sort of cookies to prevent voting twice. I guess you'd do this with cookies. I tired using a PHP form. I got a simple PHP form to email me the results but I'd rather, if possible have them post right after the person votes.

Is this something that could be done rather simply?
