3) Describe problem: I have four forms but get a "Error on page" and "form1" is not working. The other three forms do work.

Hi... can you be more specific as to which dropdown(s) don't work and what it is supposed to do? I went to your page and tried a couple and couldn't tell what the problem was.

Yes, the drop down that is under the "Minutes" column, under the "2005".
