adding a submit/decline button to form output

Hi, is there a simple way I can set a form to have a submit/decline button at the bottom of the output page?

currently the form is set up to take the user input, then display the output on the next page and automatically e-mail all of this to my e-mail. But I want the user to have the option of changing some stuff before they submit.

Can someone please tell me if there is an easy way, or better if there is a good tutorial on this somewhere?
